General Information Your Visit to Our Office - If this is your first visit to our practice, welcome. Memphis Heart Clinic is devoted to providing the most comprehensive medical care in the most comfortable environment possible.
- If you are a new patient, you will be asked to complete a set of forms that will provide us with the information we need to give you our best care. This packet will request information regarding your medical history, release of medical information, medications you are taking, a patient and insurance information. It is important that you bring all insurance cards with you so we can properly file insurance claims on your behalf.
- Please sign in at the Front Desk every time you visit the clinic. The Front Desk will let us know that you have arrived.
- Please inform us right away of any change of address, telephone number, employment, marital status or insurance.
- Before leaving the clinic please make your follow-up appointment.

- If your physician is unavailable, we have Nurse Practitioners who can see patients by appointment and on urgent basis.

Calling our Office - At times it will be necessary for you to reach your doctor or nurse. During office hours (M-F, 9:00 - 5:00 ) you should call the office. Please listen to the prompts, to ensure you are transferred to the appropriate person.
- In order to allow the doctors to devote their full attention to scheduled patients, an administrative assistant/nurse will initially handle your call. The nurse will consult with your physician and return your call.
- Please be patient with us as we receive many calls during the day. Your call will be returned as soon as possible.
- If you are experiencing an emergency, such as chest pain or shortness of breath, call 911.
- After hours, your call will be answered by our professional answering service and the doctor/nurse on call will return your call as soon as possible.
back to top Prescription Refills - Refills should be given during your office visit. If refills are needed outside this time, please call between 9:00am and 3:00pm Monday – Friday.
- All prescriptions will be phoned to your pharmacy by 6:00pm on the day of your call. Prior authorizations will require additional days for approval.
- When calling for a refill, listen to the prompts and your call will be forwarded to the prescription refill line. Please state your name, date of birth and physician name. In addition, we ask that you provide the name of the prescription, the dosage, and your pharmacy number available. We do not refill medication without a pharmacy number.
back to top Test Results - All patients are anxious to know the results of any tests that are performed. Please know that you will receive your results as quickly as possible.
- Since many of your tests will be performed at Memphis Heart Clinic, we will be able to give you the information you need promptly.
back to top Medical Records / Release of Information - We regard the doctor - patient relationship as sacred. The contents of your medical records are confidential and will only be released to outside sources within federal guidelines or upon an express written authorization.
